As we welcome patients into our Windy City Orthopedic and Sports Medicine office, we understand that you may feel concerned about COVID-19 (coronavirus). Your health is our top priority and we have put extensive measures in place to help mitigate the spread of COVID-19.
CDC Guidelines
We have modified seating plans in our waiting areas and may put you in an exam room if social distancing cannot properly be maintained. In order to minimize time in the waiting area, PLEASE fill out patient forms online BEFORE your appointment! When you call to schedule an appointment, you will be given instructions on how to login to MYPATIENTPORTAL. From there, patient forms are under the MY CHART Tab. We apologize for the length of the forms but complete medical and injury history is necessary for the treatment of your problem. We have also recently implemented telehealth capabilities for treatment of selected established patient and post-operative visits to minimize you having to physically come into the office.
Visitor Restrictions
We ask that when possible all patients arrive to their appointments alone. We understand children under 18 and patients needing an interpreter will need to be accompanied and ask that only one companion or guardian arrive with the patient.
Extensive Cleaning Process
We continue to perform rigorous cleaning of our exam rooms between patients and increased the frequency of cleaning and disinfecting high-touch surface areas. UV lights are used at the end of the day to sanitize all areas of the office.

Please call our office with your pharmacy information to refill your prescriptions electronically.
